Qualifications:

The Attendant/ Homemaker must be able to understand and carry out directions or instructions and to record notes and keep records.  Persons will be selected who are able to accomplish the essential functions required for client care, and who are tolerant of others and who are interested in and sympathetic toward caring for the sick at home.  Preference will be given to persons who have had successful work experience in caring for people in their homes.  Must be at least eighteen years old and have access to transportation, and be competent to perform personal care and meet any T.D.H. regulations required. 

Duties
  1. The Attendant / Homemaker will provide in-home care to aged or disabled persons. The care can include, but is not limited to: bathing, grooming, dressing, feeding, helping client walk, positioning and transferring, toileting, preparing meals, housekeeping chores, accompanying or arranging for transportation to the doctor, grocery shopping, reminding the client to take medication, routine hair/skin care and laundry. 
  2. Must perform all assigned tasks and activities during scheduled / authorized hours.
  3. Responsible for reporting any changes in the client’s condition to supervisor/ office immediately.
  4. Responsible for notifying the supervisor / office if for any reason client should leave their home i.e. vacation, hospitalization.
  5. Must correctly complete timesheets, sign and mail as instructed by supervisor / office
  6. Must maintain client confidentially.
  7. Must report to supervisor / office of absent from work for any reason.
Essential Functions:
  1. Accurate legible documentation on timesheets.
  2. Interacting with clients, household members and staff with courtesy and respect.
  3. Using proper body mechanics, physical maneuver clients as needed for care.
  4. Knowledgeable about safety practice, and calling for help when required.
Supervision:

The Attendant / Homemaker is responsible to the supervisor Day-to-Day communication will possibly occur with the Coordinator.  The supervisor is responsible for intervention.  Supervisor assigns all tasks and provides orientation.

Principal Functions
  1. Provides skilled nursing care to clients as prescribed by the physician and under the supervision of a Registered Nurse.
  2. Gives care in a manner, which promotes quality, continuity, and safety for the client.
Qualifications:
  1. Graduate from an approved school of vocational nursing.
  2. Current licensure as a licensed vocational nurse, preferably within the year prior to the date of application for employment.
  3. Proof of CPR certification (this must be maintained by employee).
Responsibilities:
  1. Provides nursing care to clients through implementation of the nursing care plan written by a registered nurse.
  2. Contributes to the formulation/revision of the nursing care plan under the direction and supervision of the client care supervisor or her designate.
  3. Implements orders for medications and treatments as prescribed by the client’s physician and as aspect of the nursing care plan.
  4. Assists other members of the health care team in performing specialized procedures, preparing equipment for treatments, and observing aseptic techniques.
  5. Assists client with self-care techniques, or other aspects of care as ordered by the physician or outlined in the nursing care plan.
  6. Instructs clients in self-care techniques, or other aspects of care as ordered by the physician or outlined in the nursing care plan.
  7. Works with family members and other members of the health care team to meet emotional needs of the client and family members.
  8. Maintains a safe environment for the client.
  9. Observes the client for changes in condition and reports the changes to appropriate physician and client care supervisor.
  10. Completes, maintains, and submits accurate and relevant clinical notes regarding the clients condition and all care and treatments given.
  11. Seeks and obtains sufficient information necessary to function effectively in the health care setting assigned.
  12. Follows policies and practices of Valley Care Home Health
  13. Accepts assignments and performs duties which are within the limits of preparation and experience.
  14. Fulfills the obligations of accepted shift or case assignments.
  15. Assumes responsibility to maintain and upgrade professional knowledge and skills through attendance and participation in continuing education and in-service programs.

Responsibilities:
  1. Provides professional nursing care utilizing all elements of the nursing process.
  2. Contributes to the formulation and revision of the nursing care plan in cooperation with the client care supervisor.
  3. Implements orders for medications and treatments as prescribed by the physician and all aspects of the nursing care supervisor.
  4. Initiates and applies appropriate preventive, therapeutic, and rehabilitative nursing procedures in accordance with physician’s orders and the nursing care plan.
  5. Assists client with activities of daily living as necessary while promoting the highest level of independent functioning attainable.
  6. Instructs clients and family in self-care techniques, diet, medications, and other aspects of care outlined in the physician’s orders and/or the nursing care plan.
  7. Recognizes and utilizes opportunities for health care teaching with the client and/or family members.
  8. Cooperates with family members and other appropriate persons to meet emotional needs of the client and family members.
  9. Maintains a safe environment for the client.
  10. Observes the client for changes in condition and reports these changes to the appropriate physician and Registered Nurse supervisor.
  11. Completes, maintains, and submits accurate and relevant clinical notes regarding the client’s condition and all care and treatments given.
  12. Supervises ancillary personnel and delegates responsibility appropriately
